""Homeless Outreach Program"": Mental and Substance Abuse Support; Support With Housing Assistance. The H.O.P. ""Homeless Outreach Program"": Must switch Primary care to this clinic; process will begin with patient registration in Medical or Behavioral Health services. Walk in services: monthly bus passes food bags clothing from 10-2 Fri Medicaid Enrollment Specialist Change MCO SNAP GA -Full Service CSW's are currently available in Behavioral Health and the H.O.P. ID clinic for those in H.O.P. program: Thurs 2-3:15pm at All Nations Wellness Center (6416 Zuni SE).Case manager's are currently available in Behavioral Health and the H.O.P. programs. Can also offer help in applying / appealing federal disability income cases."
